Supporting vulnerable young people in Hertfordshire for over 15 years.

To prevent homelessness by empowering individuals to make positive, informed choices. To support young people, those in crisis and individuals at risk of becoming homeless.
Why the community needs this charity

Most of our work is with young people aged 16-24 who have had difficulty in securing and maintaining safe, secure accommodation. hyh services include: Advice & Information - moving home in a planned way. Mediation - for under 16s and up to 25 years. Floating support -helping young people into independence. Crashpad - safe and secure overnight accommodation. Health - supporting 16-65s with mental health and substance misuse problems. Education - sessions on housing and homelessness.

Results and outcomes

During 2010 /11: 16-24 Mediation received 90 referrals, 78% of whom returned or remained home. Crashpad provided 1920 bed nights. Floating Support had 482 referrals and provided 43524 hours of support. HHAH saw 75 people - 82% experienced an improvement in their overall health. Under 16s Mediation worked with 133 young people and their families - 97% of young people either remained or returned home. Home Truths educated around 2900 young people.
